Our Office Policies
We look forward to seeing you on your scheduled appointment date. If you are a new patient, you can download the Initial Consult Forms and fill them out at your convenience in an effort to streamline the check-in process. We will be contacting you the day prior to your appointment to confirm the appointment date and time. When you check-in for your appointment, we will need to copy your driver's license, your insurance card(s) in order to bill your insurance company.
The mission of Sunrise City Chiropractic, PA is to provide quality health services and facilities for the community, to promote wellness, to relieve suffering, and to restore health as swiftly, safely, and humanely as it can be done, consistent with the best service we can give at the highest value for all concerned.
We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash or checks for your convenience. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient's responsibility. We will help in any way we can to assist you in handling claims.
Appointments & Cancellations
We understand that your schedule may change and that emergencies arise. If you are unable to keep your appointment, we ask that you provide us with 24-hour notice. This courtesy, on your part, will make it possible to give your appointment to another patient. .
In the case of a medical emergency outside of our office hours please call 911 or go to the nearest Emergency Room or local Urgent Care. Our office does not provide after hours service for emergencies.
